People suffering from chronic low back pain can expect no long-term relief from the popular treatment known as transcutaneous electric nerve stimulation (TENS). According to a new guideline issued by the American Academy of Neurology (AAN), treatment with TENS is not recommended for use in those having chronic low back pain that has continued for at least three months.

The new guideline comes from the Academy’s Therapeutics and Technology Assessment Subcommittee, and was recently published online in the AAN journal Neurology. The recommendation stems from an analysis of data performed by Richard Dubinsky, MD, MPH, of the University of Kansas Medical Center in Kansas City, and Janis Miyasaki, M.D., ME.d., of Toronto Western Hospital in Toronto, and their colleagues.

The TENS treatment has been used in the treatment of many disorders involving pain for more than four decades. The portable TENS devices are battery-operated, pocket-sized, units that send an electric current through electrodes applied to the skin in the area where pain is being experienced. As the unit sends an adjustable electric current through the electrodes, the patient experiences a tingling sensation in the underlying skin and muscle. The belief is that short-term relief is due to the electric nerve stimulation blocking the pain signal to the brain.

Although the researchers maintain that TENS treatment is likely effective when used to alleviate the pain of diabetic neuropathy, regarding the use of the treatment for neurological disorders, they concluded, “Further research into the mechanism of action of TENS is needed, as well as more rigorous studies for determination of efficacy.”

Close on the heels of the Ahmedabad-based Zydus Cadila beginning clinical trials of the H1N1 vaccine on humans, city-based Serum Institute of India Ltd on Thursday set the ball rolling for clinical trials of a swine flu vaccine on humans. While the vaccine will be administered at its Ahmedabad centre only next week, the process for clinical trials on 50 volunteers started on Thursday, said institute senior director Dr Rajeev Dhere.

Serum Institute is one of the companies from across the world that were contacted by the World Health Organisation for producing a preventive vaccine against the pandemic H1N1 swine flu. The WHO had supplied H1N1 seed virus to the institute, with which it developed small quantities of the H1N1 flu vaccine needed for animal trials.

The intra-nasal immunisation with the vaccine had proved to be safe on animal models and the acute and sub acute toxicity information was submitted to the Drug Controller of India (DCGI) for permission to use for clinical trials on humans.

According to Dr Prasad Kulkarni, additional director, clinical trials, Serum Institute of India, the permission has been granted and phase 1 of the trial will be conducted in Ahmedabad. "The study group of volunteers is only 50 as we want to check the safety of the vaccine. The first phase is likely to be completed by January-end, after which a pivotal study will be taken up in adults. The intra-nasal vaccine will be administered to both children and adults."

Institute officials said the application had an advantage in that the protection was simulated directly at the virus' portal of entry.

Dr V M Katoch, Director General of Indian Council of Medical Research (ICMR), said Zydus Cadilla had already started the clinical trial on humans and approvals had been given to other companies as well. Apart from Serum Institute, Bharat Biotech and Panacea Biotech were also given the seed virus for developing the vaccine.

Much is written in the fall months about fending off colds as the temperature changes, but there is no better time than in the midst of the winter months to become reacquainted with the best ways to prevent cold and flu bugs from invading your life. Some of the tips are all too familiar, but others may be pieces of advice that, incorporated into a daily routine, can help maintain health and fend off winter sicknesses.

Everyone is susceptible to cold and flu germs, no matter how tough the immune system might be. And germs affect everyone in different ways, whether it be light cold symptoms or a full-blown flu that results in missed work, neglected familial obligations, damaged exercise and diet routines, and quite a few dollars spent on medicines and tissues to assist recovery. The negative consequences of catching a bug has been complicated of late with the dangers of H1N1 and other harsher strains of the flu, which can bring a much longer recovery period or even result in hospitalization or death. Salmon and protein-rich foods also aid in creating a healthier immune system, and plenty of fluids allow the body to function properly and better fight off germs.

Stress is one of the major factors in the onset of colds and flu, simply because a stressed body is less equipped to defend itself against viruses. Reducing stress factors at work and at home during the winter months is a positive step in staying healthy, but when a stress-free work or life is not plausible, there are ways to de-stress and relieve the body of some of the anxiety. Yoga and meditation are two wildly popular methods of relaxation, though almost any form of exercise can improve the body’s chances of balancing health and stress. Ample sleep also allows the body a much-needed break from the hustle and bustle of daily life.
